Please note, this form is for the purchase of a puppy in our Bark to Basics Canines Puppy Program and not for puppies in the Bark to Basics Canine Obedience Center service dog training program. These lovely puppies are only available to become valuable pet family members. If you are interested in a service dog, please fill out the service dog information form.
